Breast augmentation for trans women is one of the most important procedures in the process of body feminisation. In the context of gender affirmation surgery, this procedure is also internationally known as breast augmentation MTF (Male-to-Female). Beyond modifying the size of the breast, this surgery allows to adapt the silhouette and contribute to a greater harmony between gender identity and physical appearance.

In the case of transgender women, there are specific anatomical features that must be taken into account during surgical planning. These include the position of the nipple, the distance between the areola and the inframammary fold and the width of the thorax. These factors influence the choice of implant and the most appropriate surgical technique.

For this reason, breast augmentation is planned on an individual basis. The selection of the type of prosthesis, its size, shape and position is made after analysing each patient's anatomy and aesthetic goals.

Breast augmentation can be part of the process of body feminisation when a person wishes to develop a more defined breast contour that is proportional to the rest of the silhouette.

In many trans women, hormone treatment can stimulate the growth of the mammary gland. However, this development may not be sufficient to achieve the desired volume or shape.

Breast implant shapes

In addition to the size, the shape of the implant influences the final result of the breast augmentation.

There are different options that allow the result to be adapted to the anatomy and aesthetic preferences of each patient.

  • Round implants: offer an even distribution of volume between the upper and lower chest.
  • Anatomical implants: are similar in shape to the natural breast, with greater projection at the lower pole.
  • Ergonomic implants: adapt to the patient's body position, adopting a more anatomical shape when standing.

In addition to the shape, different degrees of projection are considered to adapt to the silhouette and the percentage of existing breast tissue. The elasticity of the skin and the positioning of the prosthesis are also assessed to obtain a natural, attractive result in accordance with each person's wishes.

Breast augmentation surgical techniques

Breast implant placement can be performed in different positions in relation to the pectoralis muscle. The choice of plane depends on the anatomy of each patient, the existing breast tissue and the desired aesthetic result.

  • Subglandular plane: the implant is placed over the pectoral muscle and under the mammary gland.
  • Submuscular plane: the implant is placed below the pectoralis muscle, which may offer greater coverage in certain cases.
  • Dual plane: the implant is partially under the muscle in the upper pole and over the muscle in the lower pole, combining support and naturalness.

The implant is inserted through discreet incisions in different areas of the breast:

  • inframammary fold, in the lower groove of the chest
  • periareolar, at the border between the areola and the surrounding skin
  • transaxillary, through a small incision in the armpit

The choice of technique is always made after an individual surgical assessment.

Complementary procedures to breast augmentation augmentation

In some cases breast augmentation can be combined with other procedures to optimise the aesthetic result and improve the harmony of the body contour. These include:

  • breast lipofilling, which allows for improved implant coverage using the patient's own fat.
  • hybrid augmentation, combining implants and fat transfer
