The breast reconstruction is a reconstructive surgery procedure that restores the volume, shape and symmetry of the breast when the breast has been totally or partially removed or when its anatomy has been altered by previous medical treatments or interventions.
Depending on the individual case, reconstruction can be carried out using patient's own tissue (autologous tissue), breast implants or a combination of both techniques. Indications and patient profile
When to consider breast reconstruction
The breast reconstruction is often raised when the anatomy of the breast has been altered by surgery, disease or medical treatment.
This procedure can be performed after a mastectomy for breast cancer, It can also be indicated when there is loss of volume or breast deformity after breast-conserving surgery, trauma or previous interventions.
In selected patients, breast reconstruction can help:
- Restoring breast volume after a mastectomy
- Reconstructing the shape of the breast after cancer surgery
- Correct deformities or irregularities of the breast
- Improve symmetry between both breasts
- Restore harmony to body contours
Depending on the case, reconstruction can be performed immediately during breast surgery or at a later stage, when medical treatment has been completed.
The indication depends on factors such as the type of previous surgery, the quality of tissue available, the patient's general health and her preferences regarding the type of reconstruction.
Surgical techniques for breast reconstruction
There are different breast reconstruction techniques, The choice depends on the patient's anatomy, previous treatment and the desired result.
Reconstruction with autologous tissue
Reconstruction with patient's own tissue allows the restoration of breast volume without the use of external implants. It can be performed by:
- Autologous fat grafting (lipofilling)
- Tissue flaps, such as DIEP or TRAM flaps
These techniques use the patient's own tissue to recreate the breast volume and usually give a very natural result.
Breast implant reconstruction
Another option is to reconstruct the breast using silicone implants. In some cases, a tissue expander, which allows the skin to be progressively stretched to create the necessary space to place the definitive implant.
When tissue reinforcement is necessary, the following can be used acellular dermal matrix (ADM) to improve the coverage of the implant and provide greater stability to the result.
Complementary Lipofilling
He fat grafting is often used as an adjunct to breast reconstruction. This technique allows:
- Improving implant coverage
- Correcting breast contour irregularities
- Improve symmetry between both breasts
Lipofilling is usually performed in the final stages of the reconstructive process to perfect the aesthetic result.
Nipple reconstruction
Nipple reconstruction often represents the last stage of the reconstructive process. It can be performed by local flaps or using tissue from the other nipple, with the aim of recreating a symmetrical and natural appearance.
Implant and scar reconstruction
In many cases, existing scars from previous surgery can be used as an access route for reconstruction. When the available skin is insufficient, a tissue expander, which allows the skin to be progressively stretched by introducing serum until the required volume is reached.
Once adequate expansion has been achieved, the expander is replaced by a definitive silicone implant, achieving the desired volume and shape.
