Mastectomy for trans-men

Mastectomy for trans-men

Madrid | Zurich

For most trans men, the removal of breast tissue and skin represents an essential surgical step in the transition and masculinization of the body. By achieving a male-appearing chest, our patients can enjoy a full lifestyle that includes sports, swimming, or wearing clothing without the need for binders or feelings of shame. This intervention, in addition to being safe and effective, immediately and permanently elevates quality of life.

Various surgical techniques are available based on each patient’s unique anatomy, always complemented by liposuction to define the contour and achieve a more harmonious masculine silhouette. It is also fundamental to remodel the size, shape, and position of the nipples to confer a more manly appearance.

At our clinics, we focus on obtaining the best possible shape with minimal scarring. We understand that for many trans men scars can be stigmatizing, so we follow a personalized postoperative protocol to promote optimal healing and render them almost imperceptible. We also use laser technology to significantly improve scar appearance.

At Dr. Richard Fakin’s clinics in Madrid and Zurich, we combine our specialized expertise and service commitment in advanced male chest contouring surgeries, conceiving mastectomy and aesthetics as inseparable elements. We take great pride in contributing to an improved quality of life for trans men.

Surgical techniques

The surgical procedures include the removal of fatty and glandular tissue, elimination of the inframammary fold, proper skin tensioning, contour liposuction, reduction of the areola to achieve an oval, smaller shape, and its repositioning to a more lateral location. In exceptional cases, when the breast volume is very large, the free nipple graft technique is required. Each case is evaluated individually, taking into account the specific anatomy of the breasts and nipples.

What are the scars like and where are they located?

At Dr. Richard Fakin’s clinics, we always strive to minimize the size and visibility of scars. However, their extent and location depend on the size and position of the breast.

  • Periareolar:

The incision is made around the areola, blending with the surrounding skin and the hyperpigmented area. It is used in cases with small to moderate glandular tissue.

  • Horizontal:

Incisions oriented horizontally, to the sides of the nipple or, if preferred, in the inframammary fold. These are recommended for moderate or large glandular tissues.

  • Free Nipple Graft:

The scar is located in the inframammary fold, while the nipple, in an oval shape, is transplanted as a free graft to be positioned in the anatomically appropriate location. This is used in cases of large breast volume.

Postoperative care

At Dr. Richard Fakin’s clinics, we follow protocols designed to minimize postoperative discomfort. Generally, we employ a transient local sensory block, which significantly reduces discomfort during recovery.

Additionally, each patient is provided with a specific compression vest recommended for use for about six weeks. This compression system helps to shape the chest contour with a masculine appearance and facilitates skin retraction after the intervention.

For personalized advice or any questions, we invite you to schedule an appointment with Dr. Richard Fakin and take the step toward superior quality of life.
