In the case of trans women, there are anatomical particularities that differ from cis women, such as the distance between the areola and the inframammary fold, a smaller separation between the breasts, or a nipple positioned more centrally. All these aspects can be addressed through careful surgical planning, the appropriate selection of implant type and size, and a deep understanding of each patient’s expectations.

To optimize aesthetic results, complementary procedures such as fat grafting (hybrid augmentation) can often be added. In addition, gender reassignment surgery can be performed at the same time as breast augmentation, thereby reducing the number of anesthesias and hospitalizations required.

The decision regarding the brand of breast implant depends on the patient’s personal preferences and anatomical characteristics. Available implant shapes

  • Round: With a spherical configuration, it offers a uniform volume balance between the upper and lower areas of the breast.
  • Anatomical: With a natural contour, it provides greater projection in the lower pole and a result very similar to the original breast.
  • Ergonomic: It adapts according to the patient’s body position: it behaves like a round implant when lying down and adopts a more anatomical shape when upright, similar to the natural evolution of the mammary gland.

In addition to shape, different degrees of projection are considered to adapt to the silhouette and the existing percentage of breast tissue. Skin elasticity and implant positioning are also evaluated to obtain a natural, attractive result in line with each woman’s desires.

How and where are implants placed and which are the additional procedures for Breast Augmentation?

There are three main options for implant placement in relation to the pectoralis major muscle:

  • Subglandular Plane: The implant is placed over the muscle and under the mammary gland.
  • Submuscular Plane: The implant is placed beneath the pectoral muscle.
  • Dual Plane: The upper pole is positioned under the muscle while the lower pole is placed above it.

Implant Insertion Approaches

  • Inframammary Fold: The incision is made in the lower fold of the breast.
  • Periareolar: The approach is made along the border that joins the pigmented areola with the surrounding skin.
  • Transaxillary: The incision is made in the armpit area.

The breast augmentation procedure can be combined with other interventions performed simultaneously such as: breast lift and breast fat grafting to extend the implant coverage using the patient’s own tissue.

Is it necessary to replace my implants?

If there are no major changes, the patient does not suffer any type of disorder, pain nor discomfort and is pleased with their appearance, there is no need to replace the implants. The old idea that implants needed replacement after ten years is no longer true, since current implants are designed with materials that last for life and significantly reduce the long-term rate of complications (capsular contracture).
